This is where you obtain any medical documents you might need.

  • certificate  for maternity leave
  • documents needed to obtain a birth certificate
  • medical records  for insurance purposes and sick-leave
  • test results of bacterial cultures and biopsies taken during your hospital stay
  • Hospital Treatment Card with information about your diagnosis and treatment

Formalities regarding childbirth

On the second day after the birth of your child the Department of Documentation and Medical Statistics confirms the information concerning your newborn ( sex, weight, address etc) in order to prepare the necessary documents for you before you leave the hospital.

Insurance

If you plan to have state insurance cover the cost of your stay, it is important to show proof of your insurance coverage. Any document that shows  that you are covered by state health insurance will suffice, such as a record confirming your recent contributions to the state health insurance plan .

The National Health Fund accepts documents confirming coverage by state health insurance as those that are valid 30 days from:

  • the date that your certificate of employment was issued, the date of your document from the Social Security Fund (ZUS) or KRUS
  • the date that the confirmation of your insurance identification was issued
  • the date of insurance contribution – form from ZUS RMUA

Documents confirming coverage by the State Health Insurance Plan:

For patients with an employment contract

  • the  ZUS RMUA form from your employer

For self-employed patients

  • form that shows you are covered by the state health insurance plan and a record confirming your recent payments to the state health insurance plan

For patients insured by KRUS

  • a KRUS identification card and   a record  confirming your recent contributions to the state health insurance plan

For retired and disabled patients

  • senior citizen identification card or a record confirming your pension or disability payment

For unemployed patients 

  • a document from the State Employment Office confirming that you are covered by the  State Health Insurance Plan

For patients who have purchased insurance in the State Health Insurance Plan

  • your contract with the National Health Fund (NFZ) and a document from the Social Security Fund (ZUS) confirming your policy, as well as  a record confirming your recent payments

For a patient insured by a family member

  • a record of recent contributions to the State Health Insurance Plan of the family member who included you in his/her plan along with a document from the Social Security Fund (ZUS) confirming your entitlement to the policy
  • family insurance identification card with the names of family members listed and with an up-to-date stamp from your employer or from the Social Security Fund (ZUS)
  • certificate of employment from current employer
  • senior-citizen identification card with the names of family members as well as a current confirmation of pension or disability payment
  • children who are between the ages of 18 and 26 must show confirmation of their status as student or disability category.

For students over the age of 26

  • ZUS ZZA form from your university and student identification

For citizens of countries that are members of the European Union and the European Free Trade Association covered by Poland's State Health Insurance Plan during their stay

  • in the case of residency in Poland – confirmation issued by the National Health Fund
  • European Health Insurance Card (EHIC) issued in an EU or EFTA  member-state other than Poland
  • Certificate in lieu of the EHIC

How to obtain medical records

Th Department accepts written requests for medical records as well as copies , including copies of the Child's Health Record and Child's Vaccination Record. .

Copies and certified  copies of medical documents are issued for a small fee.

Patients have  the right to access their medical records pertaining to their state of health and treatment. The Law on Patients' Rights of 6 November 2008 guarantees this right.

Medical records may be issued to the patient or his legal representative or a person authorized by the patient. Following the death of a patient, the person authorized by the patient before death may have access to the records.

A written request for release of medical records is required. It may be written by the patient, patient's legal representative, person authorized by the patient, or permission granted by :

Types of access to medical records:

  •  records may be reviewed in the presence of a physician
  • records may be copied :

- unofficial copies

- certified copies
 

The cost of copies is regulated by the Law on Patients' Rights of 6 November 2008

In accordance with the above-mentioned Law, medical records are made available to :

  1.  your health insurer if required to extend your health insurance
  2. the National Health Fund, professional medical associations and national consultants   for purposes of oversight and control
  3.  Minister of Health, and  courts and criminal justice officials
  4. other public institutions if medical tests were conduct upon their request
  5. public institutions responsible for decisions regarding disability
  6. offices responsible for registering medical records
  7. insurance companies – with the patient's approval
